header shows your business name
above the form, customers see your business name pulled from cypress. this is what they recognize — make sure it's the public-facing name (not an internal nickname).

fields render in the order you added them
every field uses a clean form input — short text becomes a one-line input, dropdowns become tap-to-pick, ratings are 1–5 button rows. required fields show a red asterisk.

submit shows a thank-you (or redirects)
after submit they see your thank-you message, or get redirected to the url you set. the response immediately ties to their dragonfly profile if their email or phone matches.
