use conditional logic to hide questions

the builder doesn't yet support show-if rules. for branching workflows, build a separate form per branch and link to whichever one fits.

every field will gain a show-if rule ("only show this if a previous answer equals X"). stack multiple to build full branches without duplicating forms.
