    the qr code renders inline. download as png with the download button.

    print on a flyer or rack card

    stick the png on whatever physical surface customers see — counter sign, rack card, hand-out. they point their phone camera, the form opens, they fill it.

    tip

    qr codes don't work on every surface. dark backgrounds with low-contrast white codes can fail to scan. test the printed version with your own phone before mass-printing.
